The new one has 2 straps I can use to hold it in position over a shoulder or across your back. I made it with 4 lbs of black beans and some scrap fabric I got from freecycle.

While I had the sewing machine running, I fixed the torn corner of this fitted sheet.
The seam tore right out, so I reinforced it with a scrap of cloth.
The completed hot compress. This one has straps so it can be affixed to a desired location.
